Job Description:
- Develop and obtain approval for treatment plans from referring physician.
- Treat patients according to physician-prescribed plans.
- Assist nursing department in Restorative Aide training.
- Supervise Physical Therapy Assistants in patient care, adhering to state regulations.
- Communicate patient progress and plans with team.
- Attend Patient Care Conferences and Rehabilitation Conferences as needed.
- Participate in staff in-services training.
- Document evaluations, progress notes, and recertification in compliance with PT Board regulations.
- Record treatment changes following policies.
- Provide family/caregiver training for discharge.
- Assist in discharge planning.
- Recommend and facilitate ordering of durable medical equipment for patient mobility.
- Report equipment issues promptly.
- Foster positive relationships with patients, families, and colleagues.
Qualifications:
- Valid Physical Therapist License
